Health Connect IPA is an organization created to build and manage an independent physician network in Northern Virginia. This network of providers is dedicated to providing clinically integrated medical and healthcare services that are quality based, value driven, and patient centered, achieving “Triple-Aim” Goals*.
*The Triple Aim initiative is a framework developed by the Institute of Healthcare Improvement (IHI) that describes an approach to optimizing health system performance.
In early 2011, several independent primary care physician practices in Northern Virginia came together in an effort to advance primary care services for their local community population. The physicians and leaders of the new group had a vision of providing Virginians access to a network of providers dedicated to clinically integrated health care services that are quality based, value driven, and patient centered. As a result of this shared vision, Health Connect IPA was formed. Health Connect’s founding members include are all primary care practices in Northern Virginia: General Internal Medicine Group, Mount Vernon Internal Medicine, Fairfax Pediatric Associates, and Virginia Pediatric Group.
